Through the last two years, I have been the sole caregiver for my Father. At 95 living alone and not able to drive any longer, he became quite a handful. He insisted that he wanted to stay in his house. His health was fair and he could still live independently, until he couldn't. He became very demanding and thought I should just drop everything to attend to his every need. During… See more

My Philosophy and Approach to Wellness...
Apart from obstetrics, I practice the full scope of family medicine, from birth through the type of end-of-life decisions that my additional board certification in Hospice and Palliative Care necessitate. I believe in quality of life throughout an entire lifetime and will work as a team with you and yours to ensure that you are living your ideal life. My MDVIP-affiliated practice offers services, including comprehensive, advanced health screenings and diagnostic tests, that go far beyond those found in concierge medicine practices. It is my fervent desire to know my patients well and, by doing so, be better equipped to manage their health.
I have special interests in pediatric, adolescent, adult and geriatric health, chronic disease management and minor office procedures, to name a few. I suppose that doesn't really narrow it down, but in my office we will be able to focus on those interests that are important to you, with an emphasis on wellness and prevention. I am fluent in English and Spanish, a member of the Bayhealth Board of Directors and Medical Staff and serve Central and Southern Delaware and beyond!
I look forward to our future together.

Testimonials from patients of Dr. Hammer
I’ve been very happy with Scott Hammer as my primary cate physician for the past five years, but never more so than when I was admitted to a hospital for the treatment of pancreatitis. It was a difficult hospital stay, but Dr. Hammer was with me via phone and text throughout. He even shared some test results with me before hospital staff did and then he coordinated my post-hospitalization care… See more
Scott Hammer is an awesome person and a great doctor. We were his patients before he started with MDVIP and followed him into concierge medical care. He makes it worth it. You can tell he genuinely cares. He's always willing to go the extra mile to ensure me and my family get the best care possible. If he feels something is outside his area of expertise, he's willing to research and… See more
Dr Hammer is a super nice person. He makes time for his patients and answers all your questions. I am blessed to have him as my Doctor
I look at my life as a series of journeys. Much of my 67 years has been moving through the healthcare system in the United States with physicians that either couldn’t determine a course of effective care/treatment or was just uninterested in my case.
A very good friend of many years watched as one physician after the other either lost interest or couldn’t figure out a course of care… See more
An early June 2023 I went to Dr. Hammer‘s office along with my brother complaining of how my belly felt and he sent me to Bayhealth to get a CAT scan. The results showed that I had an area of scar tissue which led to a small bowel blockage. The blockage though caused by the scar tissue they were unsure on how it even developed and it ended up causing Pierre appendicitis. I ended up in and out… See more
My doctor is like family to me. I can't imagine not having him in my life.He has been through so many life changes with me. I can always count on him. I feel he was the reason my husband lived a little longer than he would have if Dr. Hammer hadn't seen something suspicious in a MRI. He is just all around BEST !!
Dr. Scott Hammer is available when I need him for simple questions to concerns that I have regarding how I am feeling. Whether via phone, portal or text, I can always know that I will hear from him in a timely manner - keeping my worry at a minimum.
While on a trip to hike Colorado, we tested positive for Covid. We called Dr. Hammer and within 2 hours, we had our Paxlovid prescription. We were back on trails after a 2 day respid. It took 3 days for our traveling friends’ physician to get back to them.
We are so grateful to know we have a physician who follows up so quickly and is so very thorough. We have never experienced… See more
Cancer, one of the most scary words you can hear. When I first received that diagnosis, it was Dr. Hammer who held my hand reassuring me all will be okay. Dr. Hammer set aside a time for us to dissect, discuss and plot a course for my CLL.
We spent over an hour, uninterrupted, listening to my fears, reassuring me it will be okay, and dissecting this diagnosis. Dr. Hammer had done a… See more
